Where the time goes

Why a root canal takes as long as it does

A root canal usually takes 1 to 2 hours across 1 or 2 visits, and where you land in that range depends mostly on the tooth. Very little of the time is drilling. Most of it goes to slow, careful work inside canals that are thinner than a pencil lead: locating each one, measuring its length, filing it to shape, and rinsing it repeatedly with disinfectant that needs time to act. X-rays are taken partway through to confirm that the instruments and the filling reach the tip of the root. A single-canal front tooth can sit near the short end. A molar with several canals is commonly near the long end, or is split across two appointments.

    Numbing and setup

    Anesthetic is given and left to take full effect, then the rubber dam is fitted. An inflamed tooth can be slow to go numb, and it is worth the wait before any work begins.

    Access and finding the canals

    The opening is made and each canal entrance is located. On molars a small extra canal can take real hunting, sometimes under magnification.

    Cleaning and shaping

    This is the bulk of the appointment. Each canal is measured, filed and flushed several times. More canals and more curvature mean more minutes in the chair.

    Filling and closing

    The canals are dried, filled and sealed, a final X-ray is taken, and the opening is closed with a temporary or permanent filling.

    The second visit, if there is one

    When treatment is split, disinfecting medication is left inside the tooth between appointments and the second visit completes the filling. The crown appointment that follows is separate again.

When it runs longer

Expect the long end of the range, or a second visit, if the tooth is a molar, if there is active infection or swelling that the dentist wants to settle with medication inside the tooth first, or if the canals are curved, calcified or blocked. Retreating a tooth that already had a root canal takes longer because the old filling material has to come out. Trouble getting numb, difficulty holding your mouth open, or working through an existing crown also add time. The crown is not part of these 1 to 2 hours. It has its own appointments, so ask your dentist for the full schedule.
